    COVID-19’da Romatizmal Hastalıkların Yönetimi

    Tüm dünyada Koronavirüs hastalığı-2019 (COVID-19) salgını küresel bir acil durum meselesidir. Komorbiditeleri olan, ileri yaştaki ve bağışıklık sistemi zayıf olan hastalarda ölüm riski yüksektir. Otoimmün romatizmal hastalıkları (romatoid artrit, sistemik lupus eritematozus gibi) olan hastalar, aldıkları immünosüpresif ajanlarla bağlantılı olarak zaten riskli bir bağışıklık sistemine sahiptir ve bu da onları enfeksiyonlara karşı daha duyarlı hale getirir. Tedavi kararlarına rehberlik edecek yayınlanmış kanıtlar eksiktir ve immünsüpresanların devamına ve başlatılmasına ilişkin şüpheler devam etmektedir. Romatizmal hastalıklar, COVID-19 hastalarında en yaygın immün aracılı hastalıklardır ve bu derlemede, yaygın olarak kullanılan ilaçların hastaların bu enfeksiyona duyarlılığını nasıl değiştirdiğini tartışıyoruz.

    Effectiveness of tocilizumab in a COVID-19 patient with cytokine release syndrome

    Cytokine release syndrome (CRS) is a systemic inflammatory response that can be triggered by many factors such as infections. CRS in patients with coronavirus disease 2019 (COVID-19) is life-threatening and can occur very rapidly after COVID-19 diagnosis. Tocilizumab (TCZ), an interleukin-6 (IL-6) inhibitor, may ameliorate the CRS associated with severe COVID-19 and thus improve clinical outcomes. We present a case of life-threatening CRS caused by COVID-19 infection successfully treated with TCZ. © EFIM 2020

    Evaluation of limited hand mobility in systemic sclerosis patients by using “prayer sign” and “tabletop sign”

    Objectives: To determine limited joint mobility (LJM) of the hand in patients with systemic sclerosis (SSc). Methods: LJM was evaluated with “prayer sign” and “tabletop sign” tests. LJM staging was done by Rosenbloom classification method. LJM (+) and LJM (?) patients were compared in terms of demographic findings (gender, age and duration of disease), laboratory results (ESR, CRP, anti-nuclear antibody (ANA), anti-topoisomerase I and anti-centromere), and modified Rodnan skin score (mRss) results. Results: In our study, a total of 217 patients, including 113 patients with a diagnosis of SSc, and 104 as a healthy control group with similar age and gender distribution to these patients, were included. A total of 113 (F=98, M=15) patients (limited cutaneous SSc (lcSSc=71), diffuse cutaneous SSc (dcSSc=42)) were included in this study and LJM positivity was found in 66.4% (lcSSc=38, dcSSc=37). A statistically significant difference was observed in between lcSSc and dcSSc patients according to the presence of LJM (p<0.001). There was a moderate positivity relationship between LJM and mRss (lcSSc r=0.449, p<0.001; dcSSc r=0.565, p<0.001). Conclusions: In our study, it was found that LJM staging correlated with mRss and dcSSc patients had more severe LJM findings than lcSSc. We conclude that “prayer sign” and “tabletop sign” tests used in hand evaluation in SSc patients have similar clinical results with mRss and can be simple bedside tests in daily practice.Key Points• This is the first study examining limited joint mobility (LJM) with “prayer sign” and “tabletop sign” tests in systemic sclerosis (SSc) patients.• “Prayer sign” and “tabletop sign” tests can be easily performed in daily practice.• We found Rosenbloom LJM staging correlated with modified Rodnan skin score.LJM of the hand can be a good prognostic indicator for early stage SSc patients. © 2021, International League of Associations for Rheumatology (ILAR).

    Impact of social media use on depression in patients with ankylosing spondylitis: Results of a cross-sectional study

    Introduction: Social media is a powerful tool in providing information and support for patients with chronic diseases. The aim was to assess the link between using social media and depression in a sample population of Turkish ankylosing spondylitis (AS) patients. Materials and methods: The patients completed a self-administered questionnaire, which was designed by the authors. Their demographic data, educational status, diagnosis, and favorite social network were also recorded. The Beck Depression Inventory-IA amended (revised) (BDI-IA-Turkish) was used to screen the AS patients for depression. Results: A total of 155 AS patients were included in the study. The depression scores of the patients who used the Internet (12.18 ± 6.85) and social media (12.35 ± 6.90) were compared with those who did not (27.19 ± 10.51 vs. 25.20 ± 11.66) and a significant difference (p ? 0.001) was found. Smartphone users were in the majority (73.5%). WhatsApp was the preferred social network (66.5%), followed by Facebook (52.9%), Instagram (52.3%), Twitter (19.4%) and Pinterest (5.8%). Social media users and non-users were similar in age, gender, educational level and marital status. There were no significant differences in terms of the type and duration of social media use with depression score. Conclusion: The results of this cross-sectional study confirmed that using social media can help patients with AS to cope with or be less affected by depression. Finding the most appropriate and commonly used form of social media may be an important concept for stewardship in health policies. © 2022 Asociación Colombiana de Reumatología
